The appointment form runs entirely in your browser to demonstrate validation and confirmation. It has no server endpoint, database, email service, analytics field capture, or patient record system.

Do not enter real health, dental, insurance, or other sensitive information. The form does not include a free-text health field for that reason.

A real dental practice would name its form provider, secure patient system, privacy contact, retention rules, analytics, cookies, and any applicable healthcare privacy practices here. A website privacy notice is not a substitute for a real practice’s required privacy documents.
